Event Description
|
It was reported that a patient experienced air in the patient line of a homechoice cassette without an alarm.This occurred during the setup process of peritoneal dialysis therapy.The home patient (hp) was not connected at the time of the event.During self-testing, the caregiver (cg) observed what sounded like air escaping the set and stated that the cassette was foggy and looked like it had moisture in it.Additionally, the set kept falling out of place behind the door.There was nothing unusual found during troubleshooting that would cause or contribute to the event.There was no patient injury or medical intervention associated with this event.No additional information is available.

H10: the device was received for evaluation.A visual inspection with the naked eye noted a damage and a slight cloudy appearance on the cassette.Functional testing including clear passage testing and clamp function testing were performed with no issues noted.In addition, an alarm of self-testing error 2042 was observed during functional testing.The reported condition was verified.The cause of the reported condition was due to the damage; however the cause of the damage could not be determined.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
