A lead extraction procedure commenced to remove two right ventricular (rv) leads due to non functional and redundant leads.Spectranetics lead locking devices were inserted into both of the leads to act as traction platforms to aid in extraction.It was reported that the physician successfully removed one of the rv leads without event.However, when the physician attempted to remove the second rv lead, he was unable to do so and chose to cut and cap both the rv lead and the lld contained within the lead.The physician attempted to unlock the lld from the lead, but was unsuccessful.The rv lead and lld were both cut, capped and remained in the patient's body.The patient survived the procedure.
