It was reported to boston scientific corporation that a trapezoid rx basket was used in the gallbladder during an endoscopic retrograde cholangiopancreatography (ercp) procedure performed on (b)(6) 2020.According to the complainant, during the procedure, an alliance ii handle was used in conjunction with a basket to attempt to remove a large stone.However, the tip of the basket failed to separate during the procedure.Then, the handle cannula and basket wire broke.Reportedly, the basket wire detached into the patient, and upon removing the fractured wire, it caused a mild mucosal injury to the patient.The procedure was not completed due to the event.There was no intervention or treatment performed to the patient.The patient's condition at the conclusion of the procedure was reported to be stable.
