Advanced bionics considers the investigation into this reportable event as closed.The external visual inspection revealed silicone slices on the top cover and along the lead, and the electrode was severed near the fantail and array prior to receipt.These are believed to have occurred during revision surgery.The device passed the photographic imaging inspection.System lock was verified.The electrode condition prevented some electrical tests from being performed.The device passed some of the electrical tests performed.The device passed the mechanical tests performed.This device was explanted for medical reasons.However, the failure of this device is attributed to a short circuit inside the analog chip at an electrode, and is not believed to be related to the return reason.Advanced bionics will continue to monitor failure modes of this type.This is the final report.Disclaimer: advanced bionics does not intend that this report be any admission of liability, fault or product defect.
