The results of the investigation are inconclusive since the device was not returned for analysis.In addition, no log files were received.Review of the device history record was not possible as the lot number is unknown.Based on the information received, the cause of the reported embolism and atrio-esophageal fistula could not be conclusively determined.

Event Description
|
Five weeks post pulmonary vein isolation procedure, a cerebral embolism and atrio-esophageal fistula were noted.A pulmonary vein isolation procedure was performed on (b)(6) 2020 with no complications or patient symptoms.Two or three weeks following the procedure, a stent was placed.Two weeks later, the patient presented to the hospital disoriented and was sent for a ct which revealed an air embolism in the sulci.Subsequently, the ct of the chest revealed the fistula.The patient was taken to the hyperbaric chamber for two sessions of treatment.The patient is in icu in unstable condition.There were no performance issues with any abbott devices.
