The freedom driver will be returned to syncardia for evaluation.The results of the investigation will be provided in a follow-up mdr.(b)(4) initial.
 
Event Description
The customer, a syncardia certified hospital, reported that the freedom driver exhibited a fault alarm while supporting the patient.The patient has been outside, and very active and noticed a low battery alarm, which changed to a permeant alarm when the patient went inside to change the batteries.There was no reported adverse patient impact.The customer also reported the patient was subsequently switched to a backup driver.
 
Manufacturer Narrative
The driver's alarm history was reviewed and revealed two fault codes which are caused by communication errors while operating on battery power and were most likely the customer-reported alarms.During investigation testing, it was determined that the root cause was a malfunction of the main printed circuit board assembly (pcba) due to a slightly out of tolerance speaker voltage regulator.This issue will be monitored and trended as part of the customer experience process.Syncardia has completed its investigation and is closing this file.Ce 5349 follow-up report 1.
