The results of the investigation are inconclusive since the reported device was not returned for analysis.Based on the information received, the cause of the reported event could not be conclusively determined.The instructions for use for this orbital atherectomy system states that a perforation may occur and/or require intervention as a result of the use of this device.The device history record for this oad lot number has been reviewed.No issues or discrepancies were noted during this review that would have contributed to the reported event.The device met material, assembly, and quality control requirements.(b)(4).
 
Event Description
The stealth peripheral orbital atherectomy device (oad) was unintentionally operated in a subintimal area and became caught on a tissue flap.Angiography was performed and did not reveal any complications.Balloon angioplasty was performed.Imaging was performed again and revealed a vessel perforation in the posterior tibial artery.Balloon tamponade and an external bandage were administered to resolve the issue.The patient remained stable during the procedure and was in good condition following the procedure.Per the physician, the cause of the perforation event was the oad operating in a subintimal area and becoming caught on the tissue flap, which then led to the perforation post balloon angioplasty.
