Event Description
This is filed to report the steerable guide catheter soft tip damage.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 4.The steerable guide catheter (sgc), failed to cross the septum; the sgc was removed and the septum was ballooned.The sgc still failed to cross the septum again.Upon removal of the sgc, the soft tip was noted to be flared.The sgc was no longer used.There was no adverse patient effect and no clinically significant delay in the procedure.A new sgc was used, the new sgc crossed without issues.One clip was implanted, reducing the mr to 1.No additional information was provided.
 
Manufacturer Narrative
The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot.Additionally, a review of the complaint history identified no similar complaints reported from this lot.Based on the information provided, a cause for the reported failure to advance cannot be determined.The reported deformation due to compressive stress (soft tip) was a cascading result of the reported failure to advance as the soft tip became flared while trying to advance the guide.There is no indication of a product issue with respect to manufacture, design, or labeling.
