According to the reporter, after using the catheter for approximately one (1) year, a piece of the blue connector (venous) was partially broken but no air was drawn into the catheter.Tego was used.It was also mentioned that tego lids or the plastic became brittle.The product was treated with octenisept for disinfection.There was no reported patient outcome.

Event Description

According to the reporter, during hemodialysis treatment, after using the catheter for approximately one (1) year, a piece/small part of the blue connector (venous) was partially broken (splintered) but no air was drawn into the catheter.Bloodlines and tego were being utilized with the device.It was also mentioned that the customer had a concern that the tego made the connector of the catheter brittle.There was no leak.Nothing unusual observed on the device prior to use.It was stated that they tighten the adapters by hand and octenisept was used as cleaning agent to clean the adapters.The product was treated with octenisept for disinfection.The cleaning agents were allowed to dry thoroughly prior to ointment application in the area.There was no blood loss and blood transfusion was not required.No intervention/treatment required as a result of the adapter issue.The catheter was not repaired but a repair kit was on place and it was not used up to now.They used further the product.The treatment was completed.There were no patient symptoms or complications associated with this event.The patient was recovered and no further issues.
