The results of the investigation are inconclusive since the device was not returned for analysis.Four images were submitted to product performance engineering.The photographs appeared to show the hd grid shaft damaged and entangled with the other catheters.Review of the device history record was not possible as the lot number is unknown.Based on the information received, the cause of the reported incident could not be conclusively determined.Per the ifu, catheter entrapment within the heart, or blood vessels is a possible complication of electrophysiology procedures.

Event Description
|
During an atrial tachycardia ablation procedure, while removing the hd grid from the venous sheath, one of the quadrapolar catheters became entangled with the hd grid and was pulled against the venous sheath removing it from the vein.At the time the quadrapolar catheter was insert in a sheath in the left groin, and out the venotomy in the right groin.The catheter was carefully removed from the left groin sheath with no issues.There were no adverse patient consequences.
