The companion 2 driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.(b)(4) initial.
 
Event Description
The companion 2 driver was not supporting a patient.The customer, a syncardia certified hospital, reported that the companion 2 driver did not pass the system check.
 
Manufacturer Narrative
The patient data files were reviewed and did not reveal any alarms that would indicate a device malfunction.The system check logs were also reviewed and revealed six tests performed by the customer; the last four tests did not pass the requirements for the 9-volt battery.During incoming testing, the driver did not pass the system check test for the 9-volt battery requirements, thus confirming the customer-reported issue.The 9-volt battery is replaced at every service (90-days of use interval or 2-year calendar interval).The battery that was replaced at the most recent service interval had an expiration date of march 2021.At the time that the 9-volt battery failure occurred, the battery was not expired.It is suspected that the 9-volt battery experienced an unexpected cell depletion malfunction.The root cause of the customer-reported companion 2 driver not passing the system check test was determined to be the 9-volt battery, which did not meet specification requirements.This issue will be tracked and trended as part of the customer complaint process.Syncardia has completed its investigation and is closing this file.Ce 5385 comp-(b)(4) follow-up report 1.
