The companion 2 driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.(b)(4) initial.
 
Event Description
The customer, a syncardia authorized distributor, reported that the companion 2 driver exhibited a system malfunction alarm while supporting a patient.The patient was switched to a backup driver and there was no reported adverse patient impact.
 
Manufacturer Narrative
Alarm data history review found a system malfunction alarm recorded in the driver's alarm history confirming the customer reported issue.Visual inspection of internal and external components found no evidence of damage or abnormalities.The driver passed incoming functional testing.To replicate the customer reported system malfunction, the driver was power cycled and approximately one minute after start up the driver produced a system malfunction notification and alarm.This was repeated four more times, replicating the customer reported issue.The root cause was determined to be the pressure sensor board.The root cause of the customer reported system malfunction alarm is the pressure sensor board.This issue will be monitored and trended as part of the customer complaint process.Syncardia has completed its evaluation and is closing this file.If new or additional information is received in the future, syncardia will file a follow-up mdr.Ce 5393 (b)(4) follow-up report 1.
