Endoclip was being used in operating room 9.Endoclip misfired, jammed and multiple clips fired at once.Opened another endo clip applier.Malfunctioned endoclip wrapped and placed in its original container.Endoclip sent to biomedical engineering for reporting.Manufacturer response for multiple clip applier, ligamax 5 (per site reporter).Awaiting response.
