Event Description
It was reported that during the implant of the left ventricular (lv) lead, when the delivery system was removed, the lead shifted slightly.The lead position was again adjusted.After being adjusted and without the delivery system to support the lead, it dislodged again.The lead was then removed and not replaced.No patient complications have been reported as a result of this event.
