Event Description
Physician intended to use a protégé rx carotid stent during treatment of a 40mm lesion in the patient¿s internal carotid artery.Lesion exhibited 80% stenosis.Artery diameter reported as 6mm.There was no damage noted to the product packaging prior to use.No issues were noted when removing the device from the packaging.Ifu was followed and the device was prepped without issue.Deployment issues are noted.There were not issues noted to the deployment mechanisms or device handle prior to deployment issue.It is reported the device was delivered to the lesion and was attempted to be deployed.One third of the stent was deployed but could not be fully deployed (partial deployment).The device was replaced to complete the procedure.No patient injury reported.
