The results of the investigation are inconclusive since the device was not returned for analysis.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed.Based on the information received, the cause of the reported entanglement and catheter tip fracture could not be conclusively determined.
 
Event Description
During the procedure, when a long sheath was inserted into the patient, the sheath went through the splines of the catheter and the two became entangled.An attempt was made to remove the catheter through its own long sheath while the second long sheath was still through it, however the two remained entangled.In an attempt to pull the catheter out, the metal connector at the tip of the catheter was fractured.The second long sheath was pulled back until it was no longer through the catheter and the catheter was removed successfully.The catheter was exchanged and the procedure was completed and there was no adverse patient consequence.
