(b)(4) is being used to capture the prolonged procedure required to replace this lead during initial implant.This device has not been returned; therefore, technical analysis cannot be conducted.Without a returned device it is not possible to definitively confirm how the device may have contributed to the complaint incident.If information is provided in the future, a supplemental report will be issued.
 
Event Description
It was reported that during implant this cardiac resynchronization therapy defibrillator (crt-d) exhibited high out of range shock impedance of greater than 200 ohms during shock configuration.The shock impedance was fine with the previous device.A different crt-d device was replaced and the shock configuration was successful.Another synchronous shock test was performed on the first crt-d and it was now possible to perform shock but still exhibited high shock impedance while in pocket.The physician elected to implant the first crt-d instead with therapy turned off.After further testing, the impedance showed good measurements.This device remains in service.The physician will continue to monitor.No adverse patient effects were reported.
