The device was returned, and an initial evaluation was conducted by olympus; however, investigation is ongoing.During the inspection of the device, several points of wear and tear were noted.A leak was coming from the bending section cover, a deep cut was noted on the probe unit, there were broken elements in the ultrasound image, corrosion in the scope connector, and lock damage was noted in the ultrasound connector.If additional information becomes available following device evaluation, a supplemental report will be filed.This mdr is being submitted retrospectively as part of a remediation effort related to recent system and process changes.A capa has been opened to manage the actions related to remediation of this issue and any required mdr reporting.
 
Event Description
It was reported, the ultrasonic gasrovideoscope experienced an angulation malfunction.There was no patient harm or consequence reported as a result of this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.All records indicated that the product was manufactured according to all applicable procedures and met final product release criteria.No abnormalities were found.Based on the results of the investigation, it is presumed that the reported phenomenon occurred due to device age deterioration.The device was manufactured in 2017.It was presumed that the reported phenomenon occurred due to external forces such as strong rubbing of the area during normal use including reprocess.Olympus will continue to monitor complaints for this device.
