This is 1 of 2 reports linked to mfg report number 3004608878-2021-00123.A facility reported that the mayfield triad skull clamp (a1108) was not locking tightly; it was loose and the starbursts are worn.It is unknown if there was patient involvement or if the device failure led to patient injury or surgical delay.

Device history record: the dhr documentation showed no abnormalities related to the reported failure.Mayfield triad skull clamp (a1108) was returned for evaluation.Unit received with the lock having both rotational and lateral movement and a residue buildup was present.Upon disassembly repair noted the index knob and the lock will need new components added to replace worn internal parts.Unit needs to be machined to have heli-coils added to the large starburst threads.The set screw in the swivel base was tight.The reported complaint was confirmed via inspection of the unit.The index knob was loose.No further investigation required based on the acceptability of risk and no adverse trends identified.This will be monitored and trended going forward.
