Event Description
|
Medtronic received information regarding an imaging system used for a sacroiliac and thoracolumbar procedure.It was reported that during a procedure, the o-arm system (o2) was not able to push a navigated spin to the s8 system.It was noted that the s8 showed as fully connected on the set-up page and the site was sure that they had chosen the correct stealth igs server to push to.The healthcare provider (hcp) stated that the s8 would display a pop-up message that indicated ¿unregistered o-arm exam received.¿ troubleshooting steps were performed, which included: rebooting the o2, having a navigation tag present, verifying the s8, triple checking that the right stealth was chosen to send to, trying a new ethernet cable, and pushing as non-navigated.It was also reported that the images failed to upload to dicom.The amount of delay to the procedure was unknown.There was no reported impact to patient outcome.

An s7 was used for this case.This particular s7 usually resides in our outpatient or and the ip address is set up to retrieve from our pacs system.The s7 was not able to receive data from the o-arm.They exchanged the s7 for an s8, that resides in the main or, and all was good.There was about a 25 minute delay in the case while trying to resolve the issue.
