A physician reported a perforator was used on a (b)(6) year-old female patient for a rupture aneurism clipping.The perforator's stopper did not work with 2 burr holes and disengagement failure occurred causing dural damage, minor brain contusion and brain parenchymal surface damage.Treatment with hemostasis was performed.The procedure was completed with a replacement product and no surgical delay was reported.It is unknown the manufacturer for the drill used with the perforator.It is also unknown if the drill was electronic or pneumatic, if the perforator click in place with the drill and if the recommended spring tests were performed between each burr hole.
