Event Description
It was reported that after the patient received their sixth radiation treatment, the cardiac resynchronization therapy defibrillator (crt-d) exhibited inappropriate pacing and noise/electromagnetic interference(emi) was observed on all electrograms (egm).It was further noted that the device marker channels did not match what was seen on telemetry.The physician felt that the crt-d was not functioning appropriately so the crt-d was programmed off.The patient was hospitalized and antiarrhythmic medication provided.No patient complications have been reported as a result of this event.
 
Event Description
Additional information received indicated the device was not sensing.It was confirmed that a device issue exist.It was also noted that impedance measurements have ceased, and data is not being collected.
