Event Description
|
According to the reporter, the two capsules successfully attached to the patient¿s esophagus but did not release from the delivery system.The customer broke the handle on both occasions the capsule failed to detach.There was nothing unusual about the patient or the procedure, and an endoscopy had been performed prior to the procedure and showed the esophagus to be normal.No lubricant was used to facilitate the placement of the capsule.The physician noted "hicky" marks on the esophagus from capsules #1 and #2.The patient was under anesthesia during the procedure.A third capsule was used which resolved the issue.The patient had chest pain after the procedure was finished.
