(b)(4).Batch #: v94g6e.(b)(4).Investigation summary: the product was returned for evaluation.Visual inspection and functional testing were conducted on the returned device.Upon visual analysis of the returned sample, it was determined that the el5ml device was returned with no damage in the external components.In addition, the tyvek was returned along with the instrument.In an attempt to replicate the reported incident, the device was tested for functionality.Upon testing, it was noted that the clips did not fully advance into the jaw.The device was disassembled in order to evaluate the condition of the internal components and one clip leg was pointing toward the other, creating the clip jam on the track and not allowing the clips to be fed.In addition, thirteen clips were found inside the clip track.This defect has been correlated to a manufacturing process.Based on the information currently available, a product issue was identified during the investigation of the sample received.This product issue will be addressed through our quality system.Additional complaint information monitoring for potential safety signals will be conducted through complaint trending as part of post market surveillance.A manufacturing record evaluation was performed for the finished device lot/batch number, and no non-conformances were identified.
 
Event Description
It was reported that during a laparoscopic cholecystectomy procedure, the device misfired while ligating a vessel.There were no patient consequences.
