Event Description
It was reported that the patient received possible inappropriate shocks for atrial tachycardia/atrial fibrillation (at/af) with rapid ventricular response detected by the device as ventricular fibrillation (vf).It was also noted that intermittent right atrial (ra) lead undersensing was noted.The device and lead remain in use.No further patient complications have been reported as a result of this event.
 
Event Description
Additional information received noted the patient presented to the hospital with congestive heart failure exacerbation.It was noted that the ra lead was undersensing during atrial tachycardia/atrial fibrillation (at/af) episodes and presenting electrograms (egm).It was also noted that the ra lead had low p-wave measurements.
