Event Description
|
It was reported the surgeon had impacted a g7 cup into the patients acetabulum after reaming.The cup was released from the inserter.The cup was threaded in, but possibly cross threaded.The cup was removed and placed back onto the modular handle for impaction.Afterwards it was found a small piece of threading was peeling off of the handle but it was still connected.It then broke off with a twist by the nurse.No additional information.

This follow-up report is being submitted to relay additional information.Complaint sample was evaluated and the reported event was confirmed.Visual examination of the returned product identified the threads on the tip of the device had fractured.There is scuffing on the strike plate and indentations on the shaft on the taper near the handle location.Device history record (dhr) was reviewed and no discrepancies were found.Root cause was unable to be determined.If any further information is found which would change or alter any conclusions or information, a supplemental will be filed accordingly.Zimmer biomet will continue to monitor for trends.
