Event Description
A facility reported the perforator failed to disengage when drilling the first hole during a left parieto-occipital craniotomy for left occipital lobectomy in a (b)(6) year-old child.The bit/tip of perforator suddenly entered into intracranial space and tore the dura with wide opening/laceration of the superior longitudinal sinus with significant blood loss.The situation could be controlled and the sinus was sutured.The initial planned surgery could not be performed as medical staff would like to check health state of the patient.Medical staff did postoperative ct angiography to confirm the absence of stenosis or sinus obstruction.The initial planned surgery has to be rescheduled / patient had to stay at the hospital longer period.

Updated fields: d4, d9, g3, g6, h2, h3, h4, h6, h10 the perforator was returned for evaluation: dhr - there is no indication that the production process may have contributed to this complaint.All test results passed procedural specifications.Failure analysis - the perforator unit was inspected using the unaided eye.Unit was lightly soiled but passed visual inspection and showed no observable anomalies.Ifu testing was performed with no observed anomalies.Functional was performed using the same protocol it underwent at finished goods testing prior to release.The unit was found to perform as intended and fulfilled the acceptance criteria.The complaint could not be verified through failure analysis.The root cause is undetermined and was unable to be confirmed in the complaint evaluation.
