|
One 777f8 catheter without any attached components was returned for examination.The reported event of balloon issue was confirmed.The balloon was found to be torn around circumference at both the proximal and distal bonding sites.The torn balloon piece was not returned.All through lumens were patent without any leakage or occlusion.No visible damage or inconsistency was observed from the catheter body.Visual examination was performed under microscope at 20x magnification and unaided eye.A review of the manufacturing records indicated that the product met specifications upon release.An investigation has been initiated to consider any potential manufacturing factors that may have contributed to this complaint.Complaint histories for all reported events are reviewed against trending control limits on a monthly basis, and any excursions above the control limits are assessed and documented as part of this monthly review.

Event Description
|
It was reported that the balloon did not inflate when the customer inflated the balloon on the first day of use.It is unknown if the balloon test before use was performed, or any balloon issue was detected before use there were no patient complications reported.
