Device report from (b)(6) reports an event as follows: it was reported that during a procedure on (b)(6) 2021, a washer broke during hip surgery.It was replaced with a new washer.All device fragments were removed.This report is for a washer 13.0mm.This is report 1 of 1 for (b)(4).
