Event Description
|
The syncardia companion hospital cart is a large cart with wheels into which the syncardia companion 2 driver docks.It is intended for use in the hospital during the temporary total artificial heart (tah-t) implant procedure and subsequent recovery.The companion hospital cart was not supporting a patient.The customer, a syncardia certified hospital, reported that the companion hospital cart had smoke and a burning smell when it was turned on.

During visual inspection, evidence supporting the customer-reported issue of smoke and a burning odor was found where by visible burn marks were present in the components on the inverter circuit board.Additional damage was found to the display skin and neck of the lcd display.This damage was most likely caused by impact shock as a result of rough handling.This hospital cart display is an older model and does not include the insulator on the back of the circuit board of the lcd display which was implemented to mitigate the risk of circuit component pins protruding from the back side of the circuit board coming into contact with the metal mounting bracket of the display.The root cause was determined to be the absence of the insulator combined with the impact shock likely caused a short circuit in the circuit components.This short circuit was the most likely cause of the reported smoke and electrical burning odor.This issue will be monitored and trended as part of the customer experience process.Syncardia has completed its investigation and is closing this file.Ce 5672 follow-up report 1.
