The companion 2 driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.(b)(4).
 
Event Description
The companion 2 driver was not supporting a patient.The customer, a syncardia certified hospitsl, reported that the companion 2 driver did not pass the system check.
 
Manufacturer Narrative
The customer-reported system check test failure was confirmed by patient data file review.During investigation testing, the issue was replicated only when intentionally performing the system check test with the external air remaining connected throughout the tests (ignoring the prompt to remove external air).These system check tests, performed intentionally incorrectly, led to the same failure modes observed in the patient data file.When the system check test was performed correctly, no errors or alarms were observed.The root cause of the customer-reported issue was determined to be user error in that the customer did not remove the external air when prompted during system check test.The companion 2 driver performed as intended with no evidence of a device malfunction.This issue will be monitored and trended as part of the customer complaint process.Syncardia has completed its investigation and is closing this file.(b)(4).Follow-up report 1.
