Event Description
Medtronic received a report that there was a distal tip inflation and deployment failure.The pipeline was not positioned in a bend.More than 50% of the pipeline had been deployed when it failed to open.The pipeline was resheathed two or less times.There was no additional operation performed.The stent was removed from the patient's body.The pipeline was resheathed and removed with a microcatheter.The device was prepared according to the instructions per the ifu.The catheter was hydrated per the ifu.Ancillary devices include an unknown guiding catheter, a 8fr load master guidewire, and a chikai10.

Event Description
Additional information was received indicating the location of the treated aneurysm was the internal carotid artery.The max diameter was 11mm and the neck length was 8mm.It was unknown if there was damage to the pushwire or catheter.
