Event Description
This is being filed to report the air embolism requiring intervention.It was reported that this was a mitraclip procedure to treat functional mitral regurgitation (mr) with a grade of 3.After removing the dilator from the inserted steerable guide catheter (sgc) an air bubble was noted in the left atrium (la).The sgc was steered directly above the air bubble and aspiration performed.The air bubble was removed and the procedure continued.One clip was implanted, reducing mr to 1.There was no clinically significant delay in the procedure and no adverse patient effects.No additional information was provided.
 
Manufacturer Narrative
The device was not returned for evaluation.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have resulted in this event.Based on the information reviewed, a conclusive cause for air embolism could not be determined in this complaint.The reported patient effect of emboli (air embolism) as listed in the mitraclip g4 system instructions for use (ifu) is a known possible complication associated with mitraclip procedures.The reported unexpected medical intervention appears to be due to case specific circumstance as aspiration was performed to resolve the air embolism.There is no indication of a product issue with respect to manufacture, design or labeling.
