Event Description
According to the reporter, on an open pulmonary resection, while the device was being used to separate the botaro ligament during pulmonary resection, the knife did not run.Only the staple ran.The staples were also not formed in b shape and were still in the u shape.The staple line was also incomplete proximally.The tissue was damaged.The staple line was resected and re-stapled with another reload.
