Event Description
|
Physician was attempting to use the hawkone atherectomy device during procedure to treat moderately calcified plaque in the distal, mid ,proximal popliteal artery, tibial/popliteal artery and anterior tibial artery.The vessel had moderate tortuosity and exhibited cto (chronic total occlusion-100%).The artery diameter was 6mm and lesion length was 300cm.A non medtronic(terumo destination) 6fr sheath and 0.014'' guidewire were used.The vessel was pre and post dilated.Ifu was followed.It was reported that the tip detached at the hinge pin.There was a 0.014'' wire in the sheath.The hawkone was advanced up and over and 6-7 cuts were made all the way down the sfa to popliteal.The hawkone kept catching when turned off and had to be turned on and off multiple times.When removing it the entire nosecone came off in the sheath.The entire sheath was removed and replaced.Moderate resistance was felt during withdrawal.The procedure was completed by inserting a new sheath and stenting the sfa.No patient injury.
