Event Description
Information was received from a manufacturer representative regarding a patient who was implanted with an implantable neurostimulator (ins) for urinary dysfunction/sacral nerve stim and gastrointestinal/ pelvic floor.It was reported by the manufacturer representative (rep) that when the patient had been initially implanted, the patient noted that they would be having a hip replacement surgery soon (not alleged to be related to the device/therapy,) and that the doctor should implant the ins on the opposite hip.The hip surgery day came (event date) and it turned out the patient had confused which hip they were having replaced and the ins was placed on the hip that was replaced.The day after surgery, today, they were unable to communicate with the ins.The caller stated that they had the patient's handset and that when turned on, the handset showed all black with little letters like it was rebooting or updating.Upon the handset resuming normal functionality, the caller noted all the applications were on the home screen but when they tried to connect with the mytherapy application, the ins could not be communicated with.It was noted that the caller did not have an icon to try and did not want to connect using an 8840.The caller noted the patient may still have some swelling and that there were bandages, though they were small and not really over the ins (it was noted the ins was 95% exposed from the bandages per the caller).Technical services redirected to mobility to pull logs to see if they could find anything on that end.  in an email from mobility, the mobility rep reported that they wanted to pass along that the was seeing the incision site was very close to and overlapping with the device.The rep had been wondering if this might have been a potential cause of damage to the implant per some of their discussion with technical services.It was noted that the rep hadn't asked about getting follow-up analysis for the logs yet but they may do so because of this.

Event Description
Additional information was received from a manufacturer representative (rep).The rep reported that it was unknown what impact the knee replacement had on the device but that they the inability to communicate with the ins with the ins was after the hip replacement and that the possibly likely cause may have been the bovie.It was also stated that the issue has not been resolved but the physician would like to replace the battery.No further complications were reported/anticipated at this time.
