Siemens has completed an investigation of the reported event.The root cause was determined to be a hardware error.The investigation showed that the system behavior was due to a hardware defect.The image acquisition system (ias) was unable to operate due to a defective ias computer.After a reboot, the system switched to the bypass fluoro mode, as intended for such a case.This special mode, which is a mitigation of the problem, allows the system to continue generating imaging (fluoro).However, neither acquisition nor storage or further processing of data is possible.The image quality is reduced.Such a defect can only be eliminated by replacing the affected hardware.The complete ias pc was replaced by the service organization.Because the customer's affected pc was an older model (ias bb1), it was exchanged for a compatible model of a newer type (ias bb2).Following the replacement, the system operates as specified.The occurrence rate of the error pattern and the spare parts consumption of the affected part were checked.A possible error accumulation or even a systematic error that would lead to a corrective action of the installed base could not be determined by the investigation.After detailed investigation, the incident is not classified as a reportable event as neither serious injury, death nor an unexpected, prolonged hospitalization of the patient or any other person occurred or could be expected.
