The reported oad and saline tubing were received at csi for analysis.A visual examination of the tubing revealed that the saline line barb fittings at the tubing transition were not seated, which allows the tubing to separate.At the conclusion of the device analysis, the report that the saline tubing was disconnected was confirmed due to the unseated barb fittings.The root cause was considered to be a manufacturing issue.The device history record for this oad lot number has been reviewed.No issues or discrepancies were noted during this review that would have contributed to the reported event.The device met material, assembly, and quality control requirements.(b)(4).
 
Event Description
During the procedure, the saline tubing became disconnected near the saline tubing positioners.It is unknown what caused the tubing to disconnect.A second oad was opened to use the tubing and complete the procedure as there was physician concern that air could be introduced into the patient.
