It was reported a 14mm amplatzer septal occluder was chosen for a atrial septic defect (asd) closure procedure.During the procedure the asd diameter was measured by using a "stop-flow" technique.The sizing was also confirmed by transesophageal echo (tee).The device was loaded and the user attempted to deploy the device to the defect and the la disc became deformed, presenting into a "cobra" shape.The device was removed and exchanged with a new, same sized, same batch/lot number device.The second device was deployed and the procedure was completed successfully.The patient remained stable throughout the procedure.No patient consequences reported.No additional information has been provided.
