An investigation has been initiated based on the reported information.Upon completion of the investigation, a follow-up report will be submitted.
 
Event Description
A facility reported a perforator failed to disengage: the perforator was used to insert a vp shunt.When the surgeon was drilling a burr hole, instead of stopping automatically, the perforator plunged towards the dura.An electric drill was used with the perforator.
 
Manufacturer Narrative
The perforator was not returned for evaluation; therefore, an evaluation of the device could not be performed.Lot number information has been provided; therefore, device history record (dhr) was reviewed, and no anomalies were found.The root cause(s) of the reported issue could not be determined.If additional relevant information becomes available in the future, this complaint will be reopened, and the respective evaluation performed.Trends will be monitored for this and similar issues.At present, we consider this complaint to be closed.
 
Manufacturer Narrative
The perforator was returned for evaluation: failure analysis: the perforator unit was inspected using the unaided eye.Unit was heavily soiled with organic material, no other anomalies were observed.Ifu testing was performed and noted that the hudson end was ¿sticky¿ due to a buildup of organic material.The organic material was removed and then the spring test was successfully executed.Functional testing was performed using the same protocol it underwent at finished goods testing prior to release and the unit was found to perform as intended and fulfilled the acceptance.The complaint could not be verified through failure analysis.The root cause is undetermined and was unable to be confirmed in the complaint evaluation.Although the complaint was not confirmed, potential causes of failure include user misuse.
