Event Description
As reported, the device button (start/stop button) is non functional, it cannot be switch on.The issue found at preparation for use.There is no patient involvement associated on this reported event.No user injury reported.
 
Manufacturer Narrative
The subject device was received and evaluated at rrc (regional repair center) olympus prerov.Device inspection found the front panel does not work properly, traces of liquid in connector of front panel observed.The front panel, blue bar and flat cable have to be replaced.Investigation is ongoing.This report will be supplemented accordingly following investigation.

This report is being supplemented to provide a correction for g2 and additional information based on the legal manufacturer's final investigation.G2 - checked 'other' to add the country czech republic.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the cause of the nonfunctional button occurred from a damaged flat cable.The flat cable was rusty and could no function properly.The cause of the rusted flat cable is likely due to liquid penetration inside the device.A definitive root cause of the liquid present within the device could not be identified.Olympus will continue to monitor field performance for this device.
