Event Description
It was reported that during a cryo ablation procedure, while ablating the left superior pulmonary vein (lspv), the cryo temperatures became colder than expected.The ablation was paused to allow temperatures to stabilize.Another ablation was attempted, but a pericardial effusion was noted and the patient began to decompensate.A large tear was then observed in the pulmonary vein.The case was aborted, and the patient passed away.

Event Description
Additional information indicated that during the ablation of the lspv, the ablation could not be stopped immediately, so a double stop was performed.Then the patient became hypotensive, before the effusion was noted.The effusion developed into a tamponade.An emergent sternotomy was performed to repair the perforation.The patient then went into cardiac arrest with ventricular fibrillation.Resuscitation attempts were taken to no avail.The patient died on the operating table.
