Per the gore® cardioform asd occluder instruction for use, in the section ¿potential device ¿ or procedure-related adverse events¿.Adverse events associated with the use of the occluder may include, but are not limited to: device embolism.
 
Event Description
It was reported, the physician selected a 37mm gore® cardioform asd occluder to close a 15-16mm patent foramen ovale.About 30 seconds after the device was locked and the retrieval cord removed, the device embolized to the right ventricle.The device rested in the right ventricular outflow tract.The physician did not attempt to snare the device but elected to send the patient to surgery for device removal and defect closure.The patient was doing well following the surgical closure and device removal.
