It was reported that an one-link non-dehp standard bore catheter extension set would not connect to the hub of the intravenous (iv) catheter.It was further reported that blood and saline continued to leak when the iv was getting flushed; however, there was no blood loss.The issue was identified during use of the set.A new extension set was used to continue treatment with no further issues.There was no patient injury or medical intervention associated with this event.No additional information is available.

Additional information was added to d9, h3, h6 and h10.H10: the device was received for evaluation.Visual inspection did not identify any abnormalities that could have contributed to the reported condition.All components were correctly placed and according to specifications.Functional testing was performed including clear passage and pressure testing with no issues noted.The reported condition was not verified.Should additional relevant information become available, a supplemental report will be submitted.
