An event of device deployed into cobra head was reported.The investigation confirmed the device met functional specifications when analyzed at abbott under non-physiological conditions.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications.The cause of the reported event could not be conclusively determined.
 
Event Description
It was reported that a 10mm amplatzer septal occluder was selected for an implant on (b)(6) 2021.During deployment the left atrial disc was deployed while pressed against the atrioventricular (av) valve rims due to the required angle of the delivery sheath.The device developed a cobra head shape.The device was retrieved into the delivery sheath and subsequent attempts to deploy the left atrial disc also revealed a cobra head shape.There was interaction with atrial structures during deployment.The device retained the original cobra shape regardless of interaction with atrial structures.The catheter was moved away from atrial structures following the first deployment.No angulation or kink noticed in the delivery system.The device was never detached from the delivery cable and was also tested outside of the patient but still retain the cobra shape outside of the patient.A 9mm amplatzer septal occluder was placed in the patient and following close evaluation was released effectively.The physician did not state any adverse events related to this portion of the procedure.No additional information was provided.
