According to the reporter, during a laparoscopic hysterectomy, when the uterus was manipulated, three components of the device disengaged into the cavity.One component remained inside the patient's cavity and the other two were retrieved.The surgical time was extended 30 minutes or more due to the issue.

According to the reporter, during a laparoscopic hysterectomy with total bilateral laparoscopic salpingectomy, when the uterus was manipulated and the device was being used for clamping, disarmed at the tip - the cavity forceps were disassembled into 3 parts and disengaged into the patient cavity.One component remained inside the patient's cavity and the other two were retrieved.The surgical time was extended 30 minutes or more due to the issue.Another instrument was used to resolve the concern.
