Event Description
The customer reported the evis exera iii xenon light source is blinking and there is no light source.The customer mentioned that the problem was caused by the lock and that the connector required replacement.The event occurred during preparation for use, prior to an unknown therapeutic procedure.The patient was moved to another room where the procedure was successfully performed.
 
Manufacturer Narrative
A field service engineer was unable to reproduce the event.The probable cause of the malfunction was moisture however, it dried up.The device was checked internally, tested, and replaced.The investigation is ongoing; therefore, the root cause of the reported event cannot be determined at this time, however, if additional information becomes available, this report will be supplemented accordingly.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the legal manufacturer's investigation, a root cause could not be identified.The device was not returned to the manufacturer and the phenomenon was not reproduced.However, there was a report that the repair had already been performed.The cause of the reported event cannot be conclusively determined.Olympus will continue to monitor field performance for this device.
