Event Description
It was reported that the procedure was to treat a stenosed, mildly calcified and obstructed vessel in the superficial femoral artery (sfa).It was impossible to insert the guide wire into the tip of the 5.0x80mm supera self-expanding stent system (sess) due to the delivery system being a bit mobile and not so close to the rest of the devices.There was apparent misalignment of the tip broken and the segment where the stent is placed.Another supera sess was used to complete the procedure.There were no adverse patient effects and there was no clinically significant delay in the procedure.No additional information was provided.
 
Manufacturer Narrative
The device was returned for analysis.The reported difficult to insert was able to be confirmed.The reported material separation was unable to be confirmed; however, there were noted tip damages.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no other incidents from this lot.The investigation determined the reported difficulties appear to be related to circumstances of the procedure as it is likely that inadvertent mishandling during unpackaging and/or preparation for use resulted in the noted tip damages (flared, kinked) and the noted tip lumen damages (stretched, twisted) thus resulting in the reported difficult to insert.The reported material separation was unable to be confirmed.There is no indication of a product quality issue with respect to manufacture, design or labeling.
