The device was not returned for evaluation.A review of the lot history record and complaint history of the reported lot could not be conducted because the lot number was not provided.The reported patient effects of cardiac tamponade and perforation of vessels (perforation/rupture) are listed in the xience skypoint everolimus eluting coronary stent systems instructions for use as known patient effects of coronary stenting procedures.The investigation was unable to determine a conclusive cause for the reported material rupture.Additionally, a conclusive cause for the reported patient effects, and the relationship to the product, if any, cannot be determined; however, the subsequent treatment appears to be related to the operational context of the procedure.There is no indication of a product quality issue with respect to manufacture, design, or labeling.
 
Event Description
It was reported that the procedure was to treat a lipidic lesion per optical coherence tomography (oct) with no calcification or tortuosity in the left anterior descending (lad) artery.The 3.0x15mm xience skypoint stent delivery system (sds) was prepped (air aspiration) outside the anatomy prior to use without any issues.The stent was deployed at the intended site.Upon inflation at nominal pressure, a spurt of contrast came out of the balloon causing a pericardial perforation and cardiac tamponade.The balloon and the delivery system were immediately removed and balloon tamponade was performed to treat the perforation.There was a delay in the procedure.No additional information was provided.
