Device evaluation: the device was returned for investigation, received in used conditions, inside in a plastic bag, without its original packaging and decontaminated.Visual inspection results: a black loose foreign matter was detected between ay bag and housing.Complaint is confirmed.Root cause: the foreign particles fell in the cassette during cassette assembly process and was not detected.The root cause was traced to manufacturing process.The following actions were taken: val 10038672 rev 100?analysis of foreign material? was created on (b)(6) 2021 to identify the contamination sources and will be update with the different contamination sources detected; ionizer height adjustment was made on (b)(6) 2021 to improve the ionizer performance in the place bag into the housing workstation.

It was reported the cassette had a blue piece of plastic in it.No patient injury was reported.

This remediation mdr was generated under protocol (b)(4), as a result of warning letter cms# (b)(4).Please disregard the initial report submitted with the mfr number: 3012307300-2022-01770.The report was submitted in error.
